CTLE Reporting & Certificates

To monitor overall engagement in Professional Development and to award CTLE credit to participants, the form below must be completed each time a Professional Development course is completed.

Upon submission you will immediately receive a copy of your responses. In a few days you will receive your CTLE certificate of completion.

This form is intended to be used in conjunction with the self-reporting feature in eDcotrina. For example, this form could be used to track professional learning during directed conference periods over the course of a school year. Since there is typically no certificates provided for this type of job-embedded PL, staff can use the form to track their hours, have their supervisor sign, and then upload this as a bulk entry of CTLE approved hours into eDoctrina.
