CHEO offers a variety of supports. There are three main areas of service that we access as school teams:
School-Based Rehabilitation Services
Pediatric Home and Community Care (Nursing)
Neurodevelopmental Health Team
The school-based rehabilitation department includes Occupational Therapy, Physical Therapy and Speech and Language.
This year, CHEO has expanded their Pilot Tiered Support Program that uses a UDL, whole class approach for kindergarten-grade 1 classes in all elementary schools. The OT assigned to that school will connect with the classroom teachers to support the classroom as a whole. Services for grade 2+ would remain unchanged.
Although there is a waitlist for our area, cases of extreme urgency will be prioritized. So, if this referral is connected to a major safety concern, please indicate that and connect with Josee Perrault (team lead) to let her know.
Electronic Referral Forms are available on the CHEO website found below. Follow these steps to make an SBRS O.T. or P.T. referral:
Step 1:
Review the CHEO SBRS Occupational Therapy Guidelines (linked below) to ensure the student is eligible for referral.
Step 2:
After speaking with the family, Complete the SS6a: Permission to Disclose Information Form. This form must be signed by the family or guardian before proceeding. Once signed, file it in the OSR.
Step 3:
Once you've received the signed SS6a, submit the referral via the CHEO website: School-based Rehabilitation Services - CHEO (or use the link below)

We'd like to draw your attention the the "Factsheets" which includes key information and functional strategies for parents, teachers and other adults interacting with children who have speech difficulties or stutter. The content can also be printed in a pdf format to serve as short and simple handouts!

The SBRS team continues to increase and recruit in an attempt to continue offering quality clinical and therapeutic support for the high volume of students requiring our services in occupational therapy (OT), physiotherapy (PT) and speech therapy (SLP). As such, we now include rehabilitation assistants (RA) and communication disorder assistants (CDA) to our service delivery model.
Our Master School List below that includes therapist assignment for each school. Unfortunately, we do have schools that are currently without specific designated therapist assignment. We are continuing to recruit across services in order to better support the Champlain region. Please note that we continue to support urgent safety or accessibility needs within these schools and working hard to confirm direct assignments.

With the transition to Epic, our new electronic platform for the program, Sync.com (where you previously received emails about the waitlist and active students with SBRS) is no longer accessible. If you happen to still have access to it, it does not reflect your current waitlist or active students. Please do not fret! We are working hard on creating a new gateway to access your school’s SBRS information. We'll be sure to let you know as soon as it is available.

When there is a need to access in school nursing support, education, training and/or support for staff working with students with medical needs, referrals can be made through the website link for Pediatric Home and Community Care
Virtual Diabetes Care in Schools
This team includes many services for families which can have an impact on and include support for the school. In particular, for students with an ASD diagnosis, Autism Services includes:
School Support Service
Brief consultative services that helps children and youth (of any age) become more comfortable and confident in school environments.
Urgent Response Services
12-week support for families experiencing recent escalation of high-risk behaviours.
Additionally, the Extensive Needs Service is simplifying access to the right care for families of children and youth living with multiple medical, social, physical and developmental healthcare needs. This service does not require an ASD diagnosis.
