This section provides students with direct links to:

Submit scanned PRC cases
Request documents such as:
1. Good Moral Certificate (GMC)

PROCEDURES FOR REQUEST OF DOCUMENTS

Follow these steps to request your documents efficiently:

📲 Step 1: Access the Request Form

Scan the QR Code or click the provided link to request the specific document you need.

💳 Step 2: Payment (For GMC, Certificate of Undertaking, Request for Completion of Forms for Overseas Employment/Licensing Only)

If you're requesting a Good Moral Certificate (GMC), Certificate of Undertaking, Request for Completion of Forms for Overseas Employment/Licensing Only:

Pay the required fee at the University Cashier (Ground Floor, Administration Building).
Scan your Official Receipt (OR) and upload it via the Google Form.

📄 Step 3: Prepare Required Documents

For Certificate of Undertaking (COU): Upload a scanned copy of your NOTARIZED PRC Cases.
For Request for Completion of Forms for Overseas Employment/Licensing: Upload a original softcopy of the form you need us to fill up scanned copy of TOR and RLE Summary
Ensure all uploaded files are clear and readable.

📬 Step 4: Wait for Confirmation

For GMC, COU or Certificate of Undertaking, Request for Completion of Forms for Overseas Employment/Licensing: You will be notified when your document is ready for pick-up at the CNHS Office.
For Certificate of Registration (COR) or Report of Grades: Check your email inbox (and spam folder) for digital release.

NOTE:

If you are authorizing someone to process and claim your requests, make sure to ready the following upon claiming:

authorization letter
picture of authorized person's Government Issued ID
picture of your Government Issued ID here

📥 How to Download a Form or Template

Follow these simple steps to access and download the file you need:

Click the underlined file name of the form/template you want to request.
→ This will open a Google Form.
Fill out the Google Form with the required information and click Submit.
After submitting, check your email inbox for a confirmation message.
→ The requested file will be attached at the bottom of the email.
Open the email, scroll to the bottom, and click the attached file to open it.
Once the file is open, go to the top-right corner and click the downward arrow icon ⬇️ to download it to your device.
