In this section you will find the proceudres for enrollment and the list of requirements that you need to submit.
NOTE:
For documents that need uploading, install the vFlat app (for FREE) to scan your documents. Make sure that your scanned file is clear and readable. Scan your documents on a well-lit and flat surface.

STEP 1. Submission of required documents to CNHS Dean's Office (Original and one (1) photocopy). Starting July 7, 2025, you will submit the ORIGINAL COPY to the Office of the CNHS Counselor and the photocopy to CNHS Dean's Office (Click HERE to Book an Appointment for the Submission of Requirements for CNHS Dean's Office).

LOCATION of Offices to submit the requirements:
CNHS Counselor: CNHS Building, Ground Floor, Right wing [front entry], near CNHS D Room.
DEAN'S OFFICE: CNHS Building, 2ND Floor, Right side [from front entry staircase], 3rd door.
STEP 2. Medical Examination (online) (click HERE for the procedures and requirements)
a. The Google meeting will only be necessary if there are abnormal findings in your laboratory test; thus, no need for the student to wait for the University Physician for the online meeting.
b. Medical Certificate will be released by the PSU Clinic directly to CNHS, no need to follow-up.
STEP 3. Submission of Accomplished Forms (listed below) to the designated CNHS Counselor. This is for those who submitted earlier and were not included in STEP 1. Ignore this step if you have complied with it in STEP 1.
a. Printed Admission Test Result

c. Signed Pledge of Admission and Data Privacy Content (SPA-DPC)
STEP 4. Release of ADMISSION SLIP by the designated CNHS Counselor
STEP 5. ONLINE Submission of the following documents (click HERE to submit)
a. ADMISSION SLIP (student shall keep the original copy)
b. Printed Admission Test Result
c. Signed Student Directory Form (SDF)
d. Signed Pledge of Admission and Data Privacy Content (SPA-DPC)
STEP 6. Payment of Organization Membership (online payment) (click HERE for the procedures and requirements)
a. Midwifery students (CSG, Penlight, SMO)
b. Nursing students (CSG, Penlight, SNA)
c. Keep the receipts for later submission (Release of COR)
STEP 7. Encoding of courses, assessment, and validation.
a. The College will do the #7 step.
b. For MIDWIFERY and CONTINUING Students, they will be notified online to proceed to the Accounting Office for their Assessment and pay at the Cashier’s Office to be validated
STEP 8. Release of COR (click HERE for the procedures and requirements)
a. Prepare the payment receipts for the Org Membership (New and old students)
b. Accomplished the ONLINE SUBMISSION form
c. Check your email now and then for your COR; ensure you enter your email correctly, or you will not receive anything.
IMPORTANT: The release of COR starts July 7, 2025 onwards
STEP 9. Enrollment for corporate email (for NEW STUDENTS ONLY)
a. The College will facilitate it; no need for the students to visit the IT Office.
b. The students will be notified via email once their PSU email is available.

📌 Step 1: Payment of Organization Membership Fees (Click HERE to Pay)
You will be issued an ORG clearance after you paid all the mandatory organization membership and/or complied with other organizational requirements.
📌 Step 2: Prepare Your Documents.
Fill-out the Student Cumulative Profile (click HERE) and
Student Admission Form (Click HERE).

Your Medical Certificate will be released to CNHS.
📌 Step 4: Notification to Visit the Accounting Office for the Assessment.
You will be mass notified via your batch virtual classroom or the CNHS page
📌 Step 5: Pay your school fees online, click HERE for the Payment procedure
If you are a scholar, please proceed to the Accounting Office to process your scholarship.
📌 Step 6: Release of Certificate of Registration (COR).
Once all documents are verified, your COR will be released via your corporate email.

ONLINE ENLISTMENT
Purpose of PRE-ENLISTMENT
As the College shifts to a more improved service provision to the students by processing their enrollment, PRE-ENLISTMENT is a form of consent from the students for the following reasons:
signifying that they will enroll for the coming term
allowing the CNHS personnel to process their enrollment from EVALUATION up to ASSESSMENT
Thus, if a student confirms his/her intention to enroll for the term by answering the pre-enlistment, it is implied that he/she gives his/her consent to such an arrangement; if NOT, it means either he/she will not enroll or he/she will process his/her own enrollment.
How to use the online enlistment
Prepare all the necessary documents/requirements for online enlistment (read the post above).
Identify your batch by counting from the school year you first entered the program.
Example: You entered 1st Semester, SY 2025-2026; do this: (1) 2025-2026, (2) 2026-2027, (3) 2027-2028, (4) 2028-2029. The last year of the school year is your batch or class, which is 2029.
Identify your incoming year level (for first semesters) or your current year level (for second semesters or summer).
Example: Juan is a BS in Midwifery student and is an incoming 4th year, a future member of Batch 2028. It is expected that Juan will go to BS in Midwifery 4-year Program (old curriculum) and click Level 4-First Semester.
IMPORTANT: read the instructions first before anything else. You can only enlist one time. Once your email is captured, there is no way you can enlist again unless you will request in writing to allow you to re-enter for enlistment again. Your response will be emailed to you, review it to check if all entries are correct and all requirements are submitted, if there is a discrepancy, click the EDIT RESPONSE to correct any error or discrepancy.
After the online enlistment, the college will evaluate your documents and process the necessary procedure up to assessment. You will be notified via this portal for the next steps is PAYMENT if any and the release of your CERTIFICATE of REGISTRATION.
