Enrollment

All incoming LEVEL 1 STUDENTS who are waiting for their medical certificate,

Please have the following documents accomplished, signed, and scanned, then proceed to the CNHS Counselor and bring the following documents for the issuance of your ADMISSION SLIP

1. • Student Directory Form (click button below to download and print)

   •  Signed Pledge of Admission and Data Privacy Content (click button below to download and print)

   • Admission Test Result (check your email, print it)

Once your ADMISSION SLIP is issued, submit it online so that enrollment processing begins (please go to the [Procedures] page, then accomplish STEP 5  in the enrollment procedures.

Please be guided accordingly.

Important Instructions:

1. Download the following forms/documents, print them, and fill them out. Scan the filled and signed forms before submitting them.

   • Admission Test Result (check your email, print it)

2. Submit the above documents on July 7, 2025 onwards to the Office of the CNHS Counselor located at CNHS building, Ground floor, Right Wing (near CNHS D Room). Wait for your ADMISSION SLIP to be issued by the counselor.

3. Scan your ADMISSION SLIP and submit it online together with the three (3) other documents above. Only after you submit your ADMISSION SLIP online will you be processed for enrollment (encoding, assessment, and validation). Click the button below to submit your ADMISSION SLIP

IMPORTANT NOTE: 

• The enrolling officers will process your encoding, assessment, and validation. 

• For nursing students just wait for the release of your COR (provided you have paid the mandatory memberships); 

• For the Midwifery students, please wait for the announcement to visit the Accounting Office to get your assessment and pay at the Cashier's Office, as you have to pay P2,200 for the RLE Fee for the 1 unit RLE.
