It starts at age 14: students begin their involvement in how their IEP is written, how they want their future to look, and how their transition into adulthood will be handled. The transition process is a journey, and is one that is highly important to those with special education needs. Students may require extra support in academics, social/emotional regulation, mental health, and/or other areas that impact their lives as adults. At this age, skills begin to change from classwork to future preparation.
But what about post-secondary needs? The staff in the PACES office with the Pilgrim Area Collaborative realized that even beyond high school, some students are not yet ready for adult life: working, living independently, and higher education/training are all areas that may need further support. For that reason, they began the GAP Program. They sought to make the referral process easy and streamlined, allowing both district and parent referrals for students who have fulfilled all high school requirements.
If you would like to refer your student, please complete the packets below (for independent or district referrals) or by visiting the referral page on the PAC website. Along with sending transcripts and letters of recommendation, students must complete a short writing sample and some short answers, which will help the GAP staff determine whether the program is a good fit for the student's needs and vision. Students will also be invited for a tour of the Pilgrim Area Collaborative, as well as an interview with the GAP teacher(s) and administrators.
It is encouraged that districts or parents begin the referral process in the spring prior to the academic year of GAP attendance in order to allow for adequate time to complete all parts of the referrals. Please send all completed paperwork to Deb Booth, Director of PAC Extended Services.
