Required Forms and Health Information
IMMUNIZATIONS
Current Immunization Records and Health Inventories are required for all new students.

Proof of immunizations is required by the 20th calendar day of the school year (this includes weekends). Students who have not provided proof of required immunizations by the 20th calendar day must be excluded from school until documentation has been provided.
Students entering Kindergarten or above must have 2 doses of MMR (Measles, Mumps, and Rubella) and 2 doses of Varicella (Chicken Pox vaccine).
Students entering Grade 7 must have a Tdap booster and Meningococcal vaccine.

LEAD TEST CERTIFICATE
Students entering pre-K, Kindergarten, and Grade 1, are required to have a Blood Lead Test Certificate. This is NOT required for students entering any other grades.

HEATLH RECORDS
All new students must also have a completed Health Inventory. A physical examination by a Physician or Certified Nurse Practitioner must be completed within nine months prior to enrolling in school for the first time, or within six months after starting school for the first time.
This is not only required but is so important as it helps us to know of any allergies or other health conditions (such as asthma, diabetes, seizure disorders, ADHD, Autism, etc.) that may affect your child at school. If you have not yet completed these records, please take time to have this done with your child's provider so everything is in order at the start of the school year. All new students should have a health inventory provided as soon as possible at the start of the school year.

MEDICATIONS
If your child requires an EpiPen, an Inhaler, or any other medications at school (including Tylenol, Ibuprofen, Tums, Hydrocortisone, Benadryl etc.), please have the Medication Administration Form completed by their provider and be prepared to bring the completed form and medication to school. Medications must be brought to school by a parent or guardian. Students are not permitted to bring in medications.
All medications, even over-the-counter medications, require:
1) a physician's order
2) your signed consent, and
3) for you to supply the medication in a new, unopened box or bottle.
Medication forms need to be done annually and are only valid for the current school year.
The school does not provide medications, and I cannot administer any medication without a completed Medication Form signed by the provider and the parent and unless you bring the medication.
Cough drops are permitted with parent permission but must be sent in by the parent and will be kept in the health suite and labeled for that individual child's personal use. I give out cough drops as needed for those students who have brought them in and have permission. Students are not allowed to carry cough drops with them at school.

ASSESSMENTS
If your child's provider requests their teacher to complete an Assessment to help evaluate their performance at school, please let me know. I help to coordinate the completion of Vanderbilt Assessments. Completed Assessments cannot be returned directly to the parent/ guardian and are sent directly from school to the student's provider. In order for me to send them, the parent must complete a Third-Party Consent form allowing me to release that information to the provider. Please reach out for any questions at all. COMMUNICATION
I do my best to communicate all visits to the health suite. I typically email for minor issues but will sometimes call to check-in.
I will always call if your child is sick and needs to be picked up or has an injury that may require follow-up.

ILLNESS
Please let me know if your child is absent due to illness. Please include:
when symptoms started,
what symptoms your child has,
their highest temperature if they have a fever,
if they tested positive for FLU, COVID, Strep Throat, or have been diagnosed with Pink Eye (Conjunctivitis), or other potentially contagious illness.
I track illnesses in classrooms and try to keep parents informed of illnesses going around in your child's grade.
If your child has a fever (T >100.4), they must stay home until they are fever- free for at least 24 hours without taking a fever reducing medication.
If your child is vomiting or has diarrhea, they must stay home until they have had no further episodes for at least 24 hours.
(If your child's class is having an outbreak of gastrointestinal illness (stomach bug), the health department may advise students remain home until no further episodes for at least 48 hours. If that is the case, I will let you know.)
For students with conjunctivitis (Pink Eye), they must stay out of school until they have completed at least 24 hours of antibiotic eye drops.
For students with Strep Throat, they must stay out of school until they have been on antibiotics for 24 hours before returning to school.
For students with Respiratory Illness (including flu and Covid), they must stay home and away from others until, for at least 24 hours, both are true:
■ their symptoms are getting better overall, and
■ they have not had a fever for at least 24 hours without the use of any fever-reducing medication such as Tylenol or Ibuprofen.

You can visit the Website by clicking the link or scanning the QR code at the bottom of every email I send out. Please check it out. All forms are linked on FORMS tab as well as lots of other helpful information.
It is my goal for you to feel comfortable that your child is safe and well cared for at school and that you are being kept up to date with any health concerns they have during the school day. Please don't hesitate to reach out to me for any questions or concerns you have about your child's health care needs. Although I am not in school during the summer I will check my emails periodically and do my best to respond to any questions in a timely manner.
