Q: How will I be notified if my child is sick?
A: Minor issues will be e-mailed to the parent/guardian listed on file for your child unless otherwise specified. For more serious concerns, I will reach out via phone to the contacts listed on file. Please do not hesitate to reach out if you have any questions.
Q: Can my child receive medication at school?
A: Yes, your child can receive medication at school; however, we follow the MD State process which requires:
A completed Medication Form School Medication Administration Authorization Form (marylandpublicschools.org)
The form must be filled completed by the physician or nurse practitioner, even for over-the counter medications.
The form must be signed and dated by the parent/guardian along with a contact phone number.
The medication must be sent into the school in the original pharmacy labeled box.

Q: Can my child carry his or her own medications with them?
A: No, students are not permitted to self-carry medications. This is for their safety as well as the safety of other students.Β Medications could fall out of bags and accidentally be taken by another student.Β All medications must be kept in the health suite.Β If you are having challenges getting medications for your child and need for them to be sent home daily, please reach out to me and we will work something out.Β I can pick up your child's medication every morning and return it to them prior to dismissal if needed while you are obtaining school doses.Β Please do not send your child to school with medications in their bookbag as most often they forget to let us know they are there.Β
Q: Β How often does my child need to have a History and Physical form completed?
A:Β A physical examination by a physician or certified nurse practitioner must be completed within nine months prior to enrolling in school for the first time, or within six months after starting school for the first time.Β Both parts of the Health Inventory form must be submitted with the second part being completed, and signed by the phyisican or nurse practitioner.Β If you child develops a new health condition or has a significant change in health please have the Health Inventory updated.Β If you need a health inventory form, you can click the following link: STUDENT RECORD (marylandpublicschools.org) This form is also on the FORMS page of this website.
Q: Does my child need a blood lead test certificate?
A:Β A blood lead test or signed waiver is required for all students who reside in a designated at risk area.Β This certificate is needed for students first entering Pre-Kindergarten, Kindergarten, and 1st Grade.Β The Blood Test form including designated areas can be found at the following link:Β Final_MarylandDHMHBloodLeadTestingCertificate.pdf .Β This form is alos on the FORMS page of this website.Β
Q: What vaccines does my child need for school?
A:Β OLH/SL follows the Maryland State Vaccine requirments for school children which can be found on at MD State Immunization Requirements . This can also be found on the Vaccinations page of this website.Β

Students entering Kindergarten require 2 MMR and 2 Varicella vaccinations.
Students entering Grade 7 require a Tdap and a meningococcal vaccine.Β

Q: Can my child have cough drops at school?
A: Yes, you can send cough drops in for your child with a note that he or she has your permission to have them. I will keep them in the health suite labeled for your child's personal use.
