Special Instructions for Using Copies of Forms

When using the activities below take note of when you need to copy the form first, and then attach your copy to the assignment. If you forget and attach the assignment without the Form, or attach the original, go to your Classroom, delete the assignment and try again.

If you attach the original used in your session you won't be able to edit it or view the responses.

Introduction to Form question types

“A Snow Day on the Way??”

  1. Distribute this Google Form to all your participants. There are many ways to post this to your participants.
    1. Google Classroom: Post an announcement
    2. Google Form settings

When providing this form to participants please let them know this Google Form contains the different question types that are available for a Google Form.

Activity: Exploring question types in Forms

Preparation: Make a copy of the form "A Snow Day on the Way??" in your own My Drive. When you add this activity to Classroom attach your copy of the Form to your assignment.

Using the form A Snow Day on the Way?? ask participants to fill out the form taking note of each different question type and the potential options they encounter.

Once each participant is done open the Form responses to explore the analysis provided

When all your participants complete the Google Form (you will see the total when looking at the Form with the questions tab selected) take the time to walk them through the responses for each question. To access that you open the Google Form they submitted and go to responses. From there scroll down and look through the different graphic types.

These different graphics inform teachers what the student responses are and allow a conversation about correct and incorrect responses. This is how Google Forms can be used in a formative assessment classroom setting.

Using the Quiz option in Forms

Google Forms can be used for self assessment as well. Let’s complete an example together:

Activity: Create a Google Form

Note to Facilitator - Post this as Ask a Question, Short Answer, Students can respond to each other

1. Go to Google Drive and create a form. Use at least three different question types and a video from YouTube

2. Go to the Send button and get the participants URL

3. Share the URL to your form through this question. Explore and fill out each others forms.
