Hearing and Vision Screening

This year’s hearing and vision screenings will be conducted by Mrs. Luquette and Mrs. Hons. We are grateful to our parent volunteers in years past, but we will not need additional help this year.

Screenings will begin October 2 and continue through the month of October, possibly extending into early November. Please ensure that students bring their glasses or contacts on these days. Teachers will send reminders closer to each class’s scheduled screening.

Families wishing to opt out must complete this opt-out form and return it to the Health Office by October 1st.

Screenings will be conducted for:

• Grades K–5

• Grades 7 and 9

• New students
