Technology Device Agreement Form:
Must be signed by parent/guardian each year.
These forms must be kept by the campus.
The forms should be organized and secure for easy reference in the event there is ever a question as to whether or not a student was issued a device.
After the student submits a new form for the new school year, the old form can be recycled.
Each year will have a new and updated form with the school year added at the top. Each year, all students who have or will need a Chromebook must complete and submit the new form to the office.
These forms need to be completed by October 1st of each school year.
Students who do not have a Manor ISD Device will not be issued a device until the form is completed.
Students with a Manor ISD device must return the device if a new form is not submitted by October 1st.
Optional Coverage:
Insurance is optional and is explained on the Technology Device Use Agreement Form. Online payment through the District’s Webstore (RevTrak). Cash or check is not accepted.
Device coverage is valid for the current school year, including the following summer.
Payment must be made within the first month of the new school year or enrollment.
Deadlines for coverage payments are in place to ensure coverage is paid in advance of any accidental damage.
