Clinical Health & Background Check Requirements
Documentation Required Before Beginning Clinicals
Formal documentation of a criminal background check and health and immunization records are required for clinical courses. These requirements are in addition to the immunization requirements required for admission to the University of Southern Maine (USM).
This applies to students enrolled in NUR 307 and beyond in the curriculum.
Note that students are expected to create and upload/maintain their own documents in the health and background check requirements system. Both Castlebranch and Verified Credentials have help/contact us information on their website if you run into any issues with uploading documents.
COVID - all students must be vaccinated for COVID. A booster is recommended but not required. The School of Nursing cannot guarantee clinical placements for students who are not vaccinated. This will disrupt your progression in the program and may impede or prevent your ability to graduate.
Failure to complete a required clinical rotation, including due to not obtaining a COVID-19 vaccine required by a clinical site, may result in failure of the clinical course.

Deadlines and Consequences of Missing Deadlines
Students who will take either NUR 307 or NUR 325/341 in Spring 2024 - Requirements are due NOVEMBER 1st, 2023
Note: We are updating the curriculum so starting in spring 2024 students in the fundamentals NUR 307 lab will have some direct contact with patients in the community as part of their lab experience.
Students who will take NUR 307 in Summer 2024 Requirements are due June 1st, 2024
IMPORTANT: Consequences of Missing Deadlines
If you are not in compliance by the deadline you will be dropped from all classes that require compliance and any corequisite classes.
You will not be allowed to re-register for those classes until you are fully in compliance.
If you are not in compliance by 2 weeks prior to the start of the clinical course, you will not be allowed to take the course and any corequisite courses. This will result in a delay of your graduation by a semester.
The School of Nursing has to submit documentation and paperwork to hospitals several weeks before clinicals begin and if you are not in compliance, we cannot submit your documentation. Hospitals will not accept late paperwork from us and asking them to do so damages our relationships with clinical partners.

Students will need to collect the following documentation for School of Nursing Clinical Health Requirements
Proof of COVID Vaccination (booster recommended but not required)
Measles, Mumps & Rubella (MMR)
Submit a positive antibody titer for all 3 components (lab report OR physician verification of results required)
If you get a negative titer you will need to get a booster shot. After the booster no additional titers are needed, just submit the original titer and booster proof.
IMPORTANT: Immunizations are not sufficient!
Varicella (Chicken Pox)
Submit a positive antibody titer (lab report OR physician verification of results required)

Hepatitis B - TIME SENSTIVE it takes 6 months to complete this requirement if you do not already have it!
If your Hepatitis B immunization series is MORE than one year old, ONE of the following is required:
Documentation that you received the 3-vaccine series
OR if you cannot provide your 3-vaccine series, submit a positive antibody titer for Hepatitis B (lab report OR physician verification of results required)
If your Hepatitis B immunization series is LESS than one year old, ONE of following is required:
Documentation that you received the 3-vaccine series AND a positive antibody titer for Hepatitis B (lab report OR physician verification of results required)
If your series is still in process, submit where you are in the series. New alerts will be created for you to complete the series and receive your titer
TB Skin Test
ONE of the following is required:
Documentation of 2 step TB Skin tests (1-3 weeks apart) (this is a total of 2 tests within the past year)
OR 2 consecutive years of negative annual tests (most recent test must be from within the past year and the other could be from last year)
OR T-Spot test
Or QuantiFERON Gold
If positive results, you must provide a clear Chest X-Ray OR TB Questionnaire
You must get a TB test annually
Tetanus & Diphtheria (TDAP)
Documentation of a TDAP booster within the past 10 years
American Heart Association BLS for Healthcare Provider CPR Certification
A copy of your signed American Heart Association Healthcare Provider card
Certificates OR letters from the provider are accepted temporarily for 30 days if you have not yet received your card
IMPORTANT: Online certifications that do not include an in-person test and Red Cross certifications are not accepted
Health Insurance
A copy of your health insurance card - uploaded annually
Influenza
A flu shot administered during the current flu season - A flu shot administered during the current flu season - in 2023, the due date is October 13th.
All documentation needs to be clear and legible, in pdf or jpg format. It also needs to have your full name on it and be from a medical source.
Instructions for Completing the Background Check & Submitting Health Requirement Documentation
Students who will take NUR 307 (fundamentals lab) in Summer 2024 - Requirements are due June 1st, 2024.

Renewal of Documentation with Expiration Dates
Students will need to periodically renew some documents in either CastleBranch or Verified Credentials (depending on which service you are under) in order to maintain compliance.
Yearly update of the TB test. Only one test is needed for the yearly update.
Yearly update of the flu immunization or declination form.
Yearly update of health insurance documentation.
As needed update of the Tetanus (TDAP) booster if it has expired.
As needed renewal of American Heart Association for Health Care Providers CPR (good for 2 years).
