NARRATIVE REPORT
Teachers in the Philippines, particularly under the Department of Education (DepEd), bear significant responsibility in accomplishing various school forms. These forms serve as essential documents that record and track students' academic progress, attendance, behavior, and other pertinent information. It is the teacher's duty to accurately and promptly fill out these forms in accordance with the DepEd guidelines and protocols. This involves ensuring the correctness of student information, such as names, grades, and attendance records, as well as providing detailed and insightful comments or observations when necessary. Moreover, teachers must adhere to prescribed timelines for submitting these forms to DepEd authorities, often coordinating with school administrators to facilitate their timely completion and submission.
Some of the commonly used school forms include Form 137, which contains a student's permanent academic record, including grades and attendance history, and Form 138, which provides a summary of a student's academic performance for a specific period, usually a grading period or school year.
Furthermore, school forms also include documents that record non-academic aspects of students' lives, such as health records, guidance counseling records, and disciplinary records. These forms help schools and educators address students' holistic needs by providing insights into their physical, emotional, and social well-being. Some forms also serve administrative purposes, such as enrollment forms, transfer credentials, and graduation clearance forms, which facilitate the smooth transition of students between schools and educational levels.
Additionally, teachers play a crucial role in ensuring the integrity and confidentiality of the data contained in school forms. They must handle sensitive information with utmost care, following data privacy regulations and safeguarding students' privacy rights. Teachers are also responsible for maintaining accurate records of academic assessments, grades, and other relevant data to support evidence-based decision-making and educational planning. By fulfilling their responsibilities in accomplishing school forms diligently and conscientiously, teachers contribute to the efficient management of educational data and support the overall objectives of the DepEd in promoting quality education and student welfare.
Our group of Field Study 2 (FS) students was granted by our Cooperating Teacher (CT) the opportunity to have access to, and write on some school forms that she was willing to share with us. Unfortunately, we were not given access to some school forms and it is understood due to the possible risk in information security given that these documents may contain sensitive information about a particular learner or organization. The following documents then were shared with us: SF 1, 2, 9, 10, and CS FORM 48 (DTR).
We handled these documents with utmost care and confidentiality, and we made sure that we were inputting the correct information for each set of documents.
