Forms to be completed

Please find below a link to all of the forms we need you to complete and return to us ASAP.

Please note, before completing these, you will need the following information to hand:

• SECTION 1 Pupil’s Details – Name, date of birth, name of previous school, nursery or playgroup attended, sibling details

• SECTION 2 Parental Responsibility – Full details of adults with parental responsibility

• SECTION 3 Emergency Contact – Name and contact details for an additional person to be contacted in the event of an emergency

• SECTION 4 Court Orders – If there is a court order in place for your child, please provide a photocopy to the school

• SECTION 5 Authorised persons to collect your child from school – names and contact details

• SECTION 6 Medical Information – Medical questionnaire - please have details of any medication that your child is currently taking

• SECTION 7 Developmental Information – Please have information regarding any long-standing illnesses, health problems or disabilities that your child has which mean that they have substantial difficulties with certain areas of their life

• SECTION 8 Ethnicity, Language and Religion
