Forms & Waivers

All Recreational, Mini Team, and Competitive Team athletes at Legacy T&T are required to have a signed Registration Form AND Insurance Waiver on file in order to participate in a class.  

Adult Class athletes are required to have a current Insurance Waiver on file, which is updated annually.  Please print, fill out, and sign each waiver and email them back to Legacy T&T, or bring a hard copy on your first day of class. 

**PLEASE NOTE**

All forms must be filled out to completion.  Athletes who have incomplete paperwork will not be allowed to participate in class.  For athletes under the age of 18, a parent or legal guardian must sign all forms.  For athletes age 18 and older, identification must be provided to verify age.
