Student Success

In order to move an athlete out of your athletic period you must complete the following steps:

1. Have the necessary conversation with the athlete to discuss the reason for removal.

2. Be sure to document any behavioral issues if/when they occur. 

3. Contact parents/guardian to communicate the decision for removal. 

4. Fill out the form below to with your athletes name, ID and reason for removal.

5. Keep in mind, athletes will be removed at the appropriate time to accommodate each grading period.
