Parents/Guardians Acknowledge:

• Guardian(s) have read and understand the District Student Code of Conduct and the Parent/Guardian Work-Based Learning (WBL) Guide.

• If student is accepted into a WBL program, guardian(s) consent to the enrollment of their student in a Work-Based Learning (WBL) Program in Jefferson County Public Schools.

• Transportation: Guardian(s) understand that the WBL programs do not provide transportation and are responsible for ensuring transportation to/from their student's worksite. Guardian(s) expressly release the Work-Based Learning program employer, local school, Jefferson County Public Schools, and any agents of the employer or the school system from any liability that may result from their student’s use of his/her individual transportation or any other mode of transportation their student utilizes to meet this requirement.

• Photo/Media: The WBL Coordinator and/or employer may wish to photograph participants in the program for promotional and/or educational purposes. Guardians give consent to all Work-Based Learning photographs, audio recordings, and/or video recordings taken of their minor child by Jefferson County Public Schools (JCPS) or a WBL program designee. Any photographs, audio recordings, and/or video recordings become the property of the local school/district/designee and may be used by the school, district, or others with the consent, for educational, instructional, or promotional purposes determined by the district in broadcast and media formats now existing or to be created in the future.

• Medical Treatment: Guardian(s) authorize the school and/or WBL program employer to secure emergency medical treatment. Guardian(s) will assume all financial responsibility.

• Insurance: Guardian(s) ensure their student is covered by medical/accident insurance throughout the duration of their participation in a Work-Based Learning (WBL) program.
