11th Graders - ACT Parental Consent Form

Each year, Kentucky public school students in grade eleven (11) are required by state law to take a standard college entrance exam. In addition, in Fall 2022, KDE is providing an opportunity for students in grade twelve (12) to take or retake the ACT. The Kentucky Department of Education (KDE) administers the ACT for this purpose. Below is information to help parents understand the non-test activity accompanying the actual test. The KDE Office of Assessment and Accountability requests you read this information, complete the consent portion, and return it to your student’s school.

EOS is an optional program that can help students learn about educational, scholarship, career, and financial aid opportunities at no charge. Students can opt-in to this service and provide the Interest Inventory and other non-test information.
