All students entering 6th grade need the following:
A physical examination performed within 1 year of entry into sixth grade.
An updated Kentucky Immunization Certificate that shows all age-appropriate vaccines which include a tetanus vaccine (Tdap) and dose 1 of the meningococcal (Menactra) vaccine.
A vaccine for HPV called Gardasil is optional.
