Uploading Identification Documentation

From your Home Page select Upload Documents to attached your IDs to your chart. 

Please ensure chart is up to date with your most recent

•  insurance and,

• identification card.

By making sure these are kept up-to-date we can ensure the best coverage for your services is being utilized. This is especially be important if you are considering interventional treatments - Ketamine  that require additional cooperation with your insurance carrier. 

Any other documents needed can either be shared and uploaded during a telehealth appointment or can be emailed directly to info@headinghealth.com
