To access clinical notes select either the paper icon at the top labeled RECORDS, or select the larger paper icon labeled VIEW RECORDS toward the bottom of the screen.

The home page for records gives a general overview of medical history and resources providers have sent as support through AMD. 

ALLERGIES- Will list any known allergies that have been updated through the completed patient history forms or with the provider directly. 

EDUCATION- Will list any guides and resources the provider has discussed in session to be utilized and reviewed

VACCINES- Will list any information on vaccines completed in the patient history forms or given to the provider directly

PROBLEM LIST- Will list any diagnosis discussed during sessions

CLINICAL SUMMARY- Access to a summary of previous visits

DOCS & IMAGES - Any Documents or pictures that were needed to be sent between provider and patient are found here

DOWNLOADING A SUMMARY

1. Select CHART SUMMARY to access a personal copy of the chart records for download. 

2. Choose the complete date range of what needs to be collected

3. Click VIEW. 

All visit notes that have been made available from prescribing providers or communication memos made available will be listed by date under the different sub-topics such as assessments, plan of treatment, goals, etc.

All records can be downloaded as a PDF or XML file.  

If there are any chart records missing from prescribing providers please send a message directly to your provider via the portal about the missing records.
