Completing your Registration

Welcome to Heading Health! Our mission is to change lives through transformative mental healthcare that is accessible to all.  Now that your appointment is confirmed, please review the last steps of your intake process to complete prior to your appointment.

Essential Forms and Paperwork

Your essential paperwork will come via email from registration@phreesia-mail.com, and text message from 866-540-7642. This will include a link for entering the patient portal and a link to complete your consent to treat forms, financial and privacy policies, and registration of payment on file for appointment copays.  The system will continue to prompt you up to 24 hours prior to your appointment to complete these forms before your appointment would need to be rescheduled. Once you have completed this information however, you will only be sent a final reminder of your appointment 1 hour prior to start to complete checkin and copay processing.

If your paperwork is not completed 24 hours prior to your appointment, your appointment may be canceled.

Additionally, you will receive an invitation to complete your pre-appointment questionnaires from BluePrint Health via email/text.
This platform is utilized for assisting patients and providers in monitoring your customized treatment plans and to share valuable resources as you start your healing journey. 

Patient Portal Access 

Via email, you will receive an invitation to register a patient portal. Once this has been created, you will be able to review and update your complete medical history, and directly message with our teams and providers.
