Baylor College of Medicine - Transition Medicine Healthcare 

Transition Medicine is a unique Section within the Department of Medicine. The team provides healthcare and social support services to young adults and adults with developmental disabilities who have been receiving care from Texas Children's Hospital and their affiliates. Many of the young adults/adults seen in our clinic have conditions such as Down syndrome, Spina Bifida, Autism, Cerebral Palsy, and other intellectual and developmental disabilities. 

Children's Health Insurance Plan (CHIP) 

The Children’s Health Insurance Program (CHIP) offers low-cost health coverage for children from birth through age 18. CHIP is designed for families who earn too much money to qualify for Medicaid but cannot afford to buy private health coverage. 

PATH Program at UT Physicians 

The PATH Program at UT Physicians is a unique day treatment program designed to meet the needs of adolescents with complex medical illnesses and associated emotional/behavioral challenges.

PACER - Financing Your Youth Adult's Healthcare 

Families of transition-age youth often have years of experience dealing with healthcare coverage issues and co-payments, specialized equipment, home adaptation, and other health-related expenses. However, when youth become adults, there are brand new issues to deal with. Planning ahead is critical. Young adults are at risk of losing healthcare coverage at 3 critical points in their transition to adulthood:

  • when they turn 19

  • when they graduate from high school or college, or

  • when they change jobs.

It is reassuring to know that there is national network of family health advocates and other programs that can help youth with disabilities and their families understand the health insurance options that are available to them.
