Study list petitions are requests for changes to your enrolled classes after the Registrar's Office deadlines. Study list petitions are considered retroactive if you submit them after the last day of instruction (usually Friday of Week 10 during Fall, Winter, and Spring Quarters). Pre-Public Health majors and Public Health minors should contact their College/School academic advisors for information about study list deadlines and petition processes.
Please note that we cannot process study list petitions if you have active holds that prevent enrollment transactions.
Purpose: To request to add a class to your study list after Friday of Week 3 during an academic year quarter.
Petition form: FSPH Late/Retroactive Add Petition
Deadline: Requests submitted after the add deadline but before the end of instruction are considered late. Requests submitted after instruction ends are considered retroactive.
Petition Process: The FSPH Late/Retroactive Add Petition asks students to specify the class that they wish to drop (including Department, Course Number, Lecture/Seminar/Discussion/Lab Section number, and number of units), and upload an email from the instructor that answers the following questions:
Do you object to the student adding this course after the deadline?
Has the student ever attended class? If yes, what was their first date of attendance?
Did the student take the midterm?
Did the student take the final exam?
Based on the coursework completed, what is the student's grade at this time?
You will also need to explain (1) why you were unable to enroll in this class before the deadline; and (2) why you were unable to file this petition until now.
If you are registered with the Center for Accessible Education (CAE), and the reason for your request is related to your disability, you can request CAE verification. We recommend that you request CAE verification at least one (1) week before submitting your petition.
Purpose: To drop an impacted class after Friday of Week 2, or drop a non-impacted class after Friday of Week 4, during an academic year quarter.
Petition form: FSPH Late/Retroactive Drop Petition
Eligibility: To be eligible to drop a class late or retroactively, you must not attempt or complete the final exam, paper, or project. If you attempt to complete the course in any way, you cannot drop the class.
Deadline: Requests submitted after the appropriate drop deadline but before the end of instruction are considered late. Requests submitted after instruction ends are considered retroactive.
Petition process: The FSPH Late/Retroactive Drop Petition asks students to confirm the type of drop they are requesting (i.e. late or retroactive), specify the class that they wish to drop (including Department, Course Number, and Lecture/Seminar/Discussion/Lab Section number), and upload an email from the instructor that answers the following questions:
Has the student ever attended class? If yes, what was their first date of attendance (approximate date or week)?
Did the student take the midterm?
Did the student complete all coursework and/or take the final exam and/or submit the last paper/project/assignment?
Date of the final exam, paper, or project.
Based on the coursework completed (excluding the final exam/paper/project), what is the student's grade at this time?
You will also need to explain: (1) why you were unable to drop this class before the deadline; and (2) why you were unable to file this petition until now.

Purpose: To request a change of grading basis from Letter Grade to Pass/Not Passed, or Pass/Not Passed to Letter Grade after Friday of Week 6 during an academic year quarter, or after Friday of Week 4 during a 6-week Summer session.
Petition form: FSPH Change of Grading Basis Petition
Eligiblity: This petition is rarely approved. We are unable to approve a change of grading basis for the following reasons:
Poor or superior academic performance
Receiving grades for exams, assignments, etc. after the deadline
Accidental changes to your study list
Poor judgement in choosing classes
Change of major
Illness or other extenuating circumstances
Petitions submitted for these reasons will be denied.
Deadline: Requests submitted after the grading detail deadline but before the end of instruction are considered late. Requests submitted after instruction ends are considered retroactive.
Petition Process: The FSPH Change of Grading Basis Petition asks students to confirm the type of change they are requesting (i.e. from Letter Grade to Pass/Not Passed, or vice versa) and, if they are requesting a change from Pass/Not Passed to Letter Grade, upload an email from the instructor that verifies the student's final letter grade. You will also need to explain your reasons for making this request, including: (1) the documented administrative error that occurred, or how changing the grading basis for a class in your final quarter will facilitate your graduation; (2) why you were unable to change the grading basis before the deadline; (3) why you were unable to file this petition until now.
Purpose: To request enrollment in more than 20 units during an academic year quarter (Fall, Winter, Spring), or more than 18 units during Summer Sessions.
Petition form: FSPH Excess Study List Request
Eligibility: To be eligible for excess study list units, you must meet ALL of the following criteria:
Have a cumulative (overall) GPA of 3.0 or higher
Have earned a quarter GPA of 3.0 or higher in at least 15 letter-graded units during your last completed academic year quarter
If you do not meet the above criteria, you may petition for excess study list units; however, approval is not guaranteed.
If you are an entering UCLA student and you do not have an established UC GPA, you are not eligible for excess study list units until you establish a UC GPA.
Deadline: Students can petition for excess study list units beginning on the first day of instruction through Thursday of Week 3.
Petition process: The FSPH Excess Study List Request form asks students to confirm the number of units completed in the previous academic year quarter, specify the courses they completed in the previous academic year quarter, and list the intended number of units and courses for the current academic year quarter. You will also need to explain: (1) how you would benefit from taking excess units in this particular quarter/term (include specific examples); (2) what possible challenges you foresee with this courseload, and how you plan to address those challenges; (3) the circumstances that precluded you from completing excess units during any previous quarters in which excess units were awarded (if applicable); and (4) if any of your proposed courses are currently closed and you are not enrolled, please indicate whether you have received the instructor's Permission to Enroll (PTE).
