Student Transfer/Transition Information

Use this link, Student Transfer/Transition Information , to report any student that is transitioning into a new age group, i.e. into preschool, or new building, preschool to elementary school, elementary to middle school and middle school to high school or any student that is moving to a different district.  For students transitioning into a preschool classroom out of the 0-3 age group, please complete the Audiological Assistance or DM Support for 2026-27 form also so appropriate equipment can be ordered and set-up for next fall.
