Child/Young Person Details

This is where you will enter the basic information about the child or young person for whom you are requesting support. 

Entering an email address for the young person is not mandatory and only relevant if the child is over 12. 

When accessing services, it is common practice for Young People over 12 to be our main contact, unless they wish us to communicate via a parent. 

Please ask them if they would like their email address included and make them aware that if they do, they will receive a copy of the request and all supporting documentation and will be contacted directly regarding the request.
