Reason for Request/Service Selection

Here is where you will indicate which services available through SPA you would like to request. 

You must select yes or no for each one and may select multiple services. 

When selecting multiple services, please consider the reasons for referral carefully and only select the ones that are most appropriate to the situation. 

Please note that all requests are considered and assessed individually by various triage teams, who may not feel that the services selected are appropriate to the situation. In this eventuality, requesters, parents and children and young people (where appropriate) will always be made aware of the decision not to allocate a requested service  and will be provided with clear explanations for this decision. 

In addition, there may be other services not on this list that the triaging teams feel are more appropriate and which requesters will be signposted to in this case. 

Please include the expectations of the child, young person or family in the last box on this page. This is extremely helpful for our triaging teams and panels.
