Parent Details

At the start of this page you will be asked about the child or young persons age. 

Parent details will only be sought when the young person is under the age of 16. 

If the young person is 12 or over, they must give consent for their parent's details to be included. 

If the young person is under 12, the parent's details must be included and the parent must have consented to the request. 

Please include an email address if possible as this will be our main method of post submission communication. 

If an email address is not available, we will communicate by post. 

There is an option to add an additional parent. If an additional parent is included, they will receive the same information as the main parent. 

Please note - All parents who's details have been provided will receive a copy of the completed form by email or post.
