The transition from Early Intervention to School Age begins each school year around the November/December time frame. During this process, the early intervention service coordinator will collaboratively work with the school district to support a smooth transition to school age programming.

November/December:
The school district will meet with the Early Intervention Director & Service Coordinator through the Hazleton Preschool Program.
January:
Meet the District Night- This is an informal informational session for parents to meet a district representative to discuss transition processes and answer any questions you may have regarding school age procedures.
February: All meetings will be completed by February 28th.
Notice of Your Child's Transition to School Age Meeting will be scheduled with the early intervention service coordinator. These meetings will be held within the school district.
Parents will be given "Intent to Register" form. (Children do not have to attend the Intent Meeting)
If you intend to enroll your child in Kindergarten the following year, you must go on-line to register your child in the school district. (See link above)
April:
If an evaluation is decided upon at the Intent to Register Meeting, a permission to test will be sent home in the mail after the registration process is completed on-line.
THE PERMISSION TO TEST MUST BE SIGNED TO BEGIN THE EVALUATION PROCESS.
Once the signature is received in the Special Education Office-the evaluation will be completed within 60 school days.
May/June:
Eligibility meetings will be held to determine if your child qualifies for any special education services within one of the 13 categories within the state of PA.

4. Visual impairment (including blindness)
5. Emotional disturbance
6. Orthopedic impairment
7. Autism
8. Traumatic brain injury
9. Other health impairment
10. Specific learning disability

13. Multiple disabilities
Programming will be discussed at this meeting for the upcoming Kindergarten school year.
