RE-EVALUTION PROCESSES
Re-evaluations are conducted every 2-3 years based on the student's primary disability category.
This year, we have transitioned to electronic communication as the PDE has updated the process and forms. If your child is due for their biannual or triannual re-evaluation, you will receive an email from the Special Education Administrative Assistant informing you about the upcoming re-evaluation for your child.
What you will receive via Email:
Prior Written Notice for Re-evaluation
Parent Input Google Form RR Parent Input Form
Procedural Safeguards Notice
The Request for Consent will come from webserver@iepwriter.com -this is where you e-sign and consent to additional testing.
Additional Information:
On May 1st, 2023, PDE Bureau of Special Education (BSE) announced that it had revised the recommended PWN/RFC form.
Beginning Nov. 1, 2023 as part of the re-evaluation process parents will receive two forms.
Forms are available on the PDE Link: Under the Prior Written Notice and Request for Consent for Reevaluations Review
