Portage Park Families-

Please use the blue links below to access required registration forms for the new school year. There are 2 sets of forms. The first set of forms needs to be printed and completed for EVERY student who attends Portage Park. (If you have multiple students, you will need to print out and complete copies of the form for every student in your home.)

The Family Income Form must be completed once per family. Please print this form out and complete it once for your family that attends Portage Park. The Family Income Form can be found here: Portage Park Family Income Information Form

These forms must be printed and signed before returning them to school. Due to the building being closed for Remote Learning, we are unable to print these forms for families. Once you've printed and signed the forms, you may scan the signed form and email it to Ms. Hoffman at cahoffman@cps.edu, or drop the form in our drop off box located outside of door 1 where lunches are picked up. The forms may be dropped off M-F from 8:00am-4:00pm.
