Teamsters members' medical and vision plans are managed through NorthWest Administrators (NWA). The CUSD Benefits Department only manages the dental plan for Teamsters members. If you have any issues with your medical/vision please contact NWA; if you have any issues with your dental please contact the Benefits Department. Below you will see more detailed information on the medical, vision, and dental plans for Teamsters members.
Medical Plan
Kaiser HMO plan is the only medical plan that is available for Teamsters members. As a Teamsters member, you are able to add any eligible dependent onto your medical plan. You will need to complete the two forms below and provide the required documents to the Benefits Department and NWA so that your dependents will be added. The rates and summary of benefits for this particular Kaiser plan can be found below.
Vision Plan
VSP is the vision plan that is available to Teamsters members. You are able to add any eligible dependent to your vision plan. You will need to complete the two forms below and provide the required documents to the Benefits Department and NWA so that your dependents will be added. The summary of benefits for this particular Kaiser plan can be found below.
Note: To add a dependent to your vision plan you need to add them to your medical plan as well. Since NWA manages the medical and vision plans for Teamsters members, they require the dependent to be enrolled in both.
Dental Plan
Since the CUSD Benefits Department manages the dental plan for all Teamsters members, the dental options are the same for all employees. You can find the dental plan options and details here.
Want to add or delete dependents?
As a Teamsters member, you can make changes to your medical/vision plan year-round. You will need to schedule an appointment with American Fidelity to make the changes.
*You can only add or delete dependents from your dental plan during open enrollment!!!*
Opt-Out of benefits | Do you already have benefits through your spouse or another job?
Compton USD offers all benefit-eligible employees the option to waive all of their benefits to receive the Cash-In-Lieu payment. Teamsters members must complete the Opt-Out form and provide proof of their active coverage. Once the form is complete, the employee will need to send the completed form and proof of coverage to NWA during April and May. If the form and proof of coverage are not completed and turned in to NWA by the end of May, the employee will be automatically denied.
