CUSD Employees
Regular, full-time employees working at least 30 hours per week qualify for health benefits. The District contributes a maximum amount, in accordance with the Collective Bargaining Agreements towards employee health benefits for full-time employees working at least 30 hours per week; pro-rated for part-time employees.
If you choose to enroll your family members, you may select one of the following:
Two-Party Rate: Self and one eligible dependent (Dependents Age out at age 26)
Family Rate: Self and two or more eligible dependents (Dependents Age out at age)
Once you’ve added two or more dependents, additional dependents will not change your premium amount. You can add dependents at the time of your enrollment, during our annual Open Enrollment, or with a qualifying event within 60 days following the life event. Dependents are covered until age 26 and it is your responsibility to notify the Benefits Department.
Dependents
Your spouse/domestic partner and children can be enrolled in our Medical, Dental, Vision, Voluntary Life and AD&D Insurance. Eligible Dependents include:
Your legally married spouse.
Your registered domestic partner (as defined by the state of California).
Your children, stepchildren, or children of your registered domestic partner are eligible for benefits, regardless of their marital or student status. For medical benefits, your children’s eligibility lasts until the end of the month in which they turn 26 years old, while for dental and vision benefits, it lasts until the end of the month in which they turn 25 years old.
Any children for whom you are required to provide coverage under a Qualified Medical Child Support Order.
Your unmarried children, step-children or children of your registered domestic partner of any age, if they are incapable of self-care due to a physical or mental disability.
New Hires — Required Documents to Enroll Dependents in Health Benefits
You must provide the following documents to enroll your eligible dependents in a CUSD health plan
Spouse: A copy of your most current IRS tax filing if married over one year; a copy of your marriage certificate if enrolling a spouse within one year of your marriage date
Domestic partner: A copy of the filed Declaration of Domestic Partnership with the state of California
Children: you must provide one of the following:
Biological Child: Government-issued birth certificate
Adopted Child: Government-issued adoption certificate
Stepchild: Government-issued birth certificate AND If married more than one year, Employee’s 2021 or 2022 Federal Tax Return (main page of Form 1040 or 8879). If married less than one year, Employee’s Government-issued Marriage Certificate. Employee’s Registered Certificate of Domestic Partnership issued by the State of California
Legal Guardianship: Government-issued birth certificate AND Government-issued Letters of Guardianship
Child with a Disability: Government-issued birth certificate AND Notice of disability determination from medical carrier (within 6 months) AND Notice of disability determination from the Social Security Administration (within 6 months)
Important Note: Enrolling an ineligible person as your dependent can result in paid claims being reprocessed and charged to you.
