If the injury is life-threatening, call 911
Immediately report the incident to your supervisor
Call 1-844-322-4668 within 24 hours of injury
If you seek medical care following a work injury, be sure to send a copy of the doctor's note to Human Resources at humanresources@chipfalls.org

Before seeking care at the Cardinal Healthy Clinic you must call 1-844-322-4668 to file an injury claim.
If you are injured at work and would like to seek care at the Cardinal Clinic you are able to do so. You do not need to be an insurance participant to be seen for a worker's compensation injury.
