There are two ways to start the process:

2. You can complete  the Authorization Form  and start the process without talking with someone

What to Expect When You Call:

• If you choose to call, a ReaLiving associate will take your call. 

• They will ask you some general questions (phone number, employer, DOB). 

• They will ask about what you are experiencing to prompt the call, but you do NOT have to share that information if you're not comfortable. 

• The person you are talking with will NOT be the service provider, so you don't have to share any details with them, just be sure they match you to the services you are wanting.

• They will then help you to find a provider to work with as soon as possible. 

What to Expect When You Use the Website:

• Go to this link and enter the required information.

• Your information will be reviewed by a REALiving Associate and they will reach out to you ASAP to connect you with a provider.
