Parental Consent Form

If you are under 18 years of age, we need this form filled out by your parents AND notarized before your first day of work.

In addition to the parental consent form: if you are under 18 years of age and homeschooled, we need a letter from your homeschool association stating that you are homeschooled and allowed to work during normal school hours.
