Complete by January First.
A form with backup documentation will be sent to you by Crystal Blumich.
Please sign and date the form and return the original (with supporting documentation) to Crystal Blumich.
Crystal will upload the form and documentation. (Element 5)
Schools complete the self-monitoring instrument once each year. This instrument allows schools to see their progress and to identify areas that may need additional focus. It also gives schools the opportunity to review the documentation that has been collected for the current year.
Documentation needed:
Completed self-monitoring and desk audit with detailed notes. (The link will force you to make a copy for your school.)
