Parent and Family Engagement Event (PAC 3)-(if not completed previously)
Participating schools implement strategies to build parent capacity for supporting student learning at home. Engagement events include one or more of the following:
providing parents with information and/or activities to help students at home
providing parents with training on literacy and using technology, as appropriate, to foster parent involvement
providing other reasonable support for parental involvement activities as parents may request
Engagement events allow parents to become equal partners with the school and build communication between the home and the school. Title I schools have a minimum of two schoolwide meetings/events per year. (Element 5, 6, 11)
Documentation needed:
PAC 3 Form (will force you to make a copy of the form for your school use).
Evidence of advertisement. Include examples of information shared via social media, newsletters, websites, or other means, to build parent capacity.
Include a copy of an alternative language/format for advertisement and meeting materials as appropriate for parents with limited English proficiency and/or parents with disabilities.
Dated records from the meeting or events at which parents were provided information, activities, and/or training, including agenda, meeting notes, and sign-in sheets showing affiliation and position of participants and signatures. Minutes should include detailed notes of parent comments/input.
Slide presentation and other materials used and shared during the event. If there are different materials used based on grade levels and/or content areas, include a representational sample and include descriptions in the meeting minutes of additional materials provided. If you use a video, include a link to the video in PAC 1 form. Video may be uploaded to the documentation folder separately.
Signed copy of the Parent Activity form approval for Title I funds spent. (Must be signed by the principal and Title I Director.)

Parent Survey
Parent surveys assist schools in gathering feedback and input for the development of parent and family engagement plans and activities, school improvement plans, and other related plans. Results of the parent surveys may be included in the Comprehensive Needs Assessment.
Documentation needed:
Parent Survey - templates are available through these links: English and Spanish. The links will force you to make a copy.
The survey should be school-specific. Send to Jeni McNulty at jmcnulty@caldwellschools.com for approval before distributing to parents.
Dated records from the meeting or events at which parents and other stakeholders were involved in the revision/development of the survey, including agenda, meeting notes, and sign-in sheets showing affiliation and position of participants and signatures. Minutes should include detailed notes of parent comments/input and a description of any changes made to the survey
Include a blank copy of the original English survey and information on the alternative languages/formats as appropriate for parents with limited English Proficiency and/or parents with disabilities. Include a copy of any cover letter(s) used in the distribution.
Include a description of electronic distribution, if appropriate.
Include a tally of parent responses received from all sources (paper and/or electronic).

Parent Advisory Council Meeting (PAC 4)
Annual schoolwide meeting to gather input from parents and stakeholders during planning for the next school year. This meeting is held near the end of the current school year. During this meeting, parents and families will have the opportunity to discuss possible uses of Title I funding during the next school year; review and suggest improvements to the Parent Family Engagement (PFE) Policy and the Student Parent School Compact; and provide input for school improvement planning. (Elements 1,3, 6, 11)
Documentation needed:
PAC 4 Form (will force you to make a copy of the form for your school use).
Evidence of advertisement.
Dated records from the meeting including agenda, meeting notes, and sign-in sheets showing affiliation and position of participants and signatures. Minutes should include detailed notes of parent comments/input.

reviews/revises Student Parent School Compact;
reviews/revises/improves Parent Family Engagement (PFE) Policy;
asks for input on Title I parent survey, Comprehensive Needs Assessment, and/or, school improvement objectives;
explains how Title I funding was used to supplement the educational program in your school during the current school year and asks for input for the next school year.
Include a copy of an alternative language/format for advertisement and meeting materials as appropriate for parents with limited English proficiency and/or parents with disabilities.
Documentation does not need to include the completed/revised PFE Policy and/or the Student Parent School Compact. Those documents will be uploaded in separate files during the next school year. The PAC 4 documentation includes references to those items.

Report Card Attestation for 2023-2024
Each parent is provided information annually detailing the academic achievement of the school. The NC School Report Card is used to provide parents with the most recent school information. Parents are given information on how to access the website for the school report card. Parents without internet access must be provided an alternate means of accessing the report card or provided with a copy or screenshots of the snapshot/summary. (Element 6)

Copy of web page showing access to the school report card. Include a copy of an alternative language/format for a letter, flyer, or materials as appropriate for parents with limited English proficiency and/or parents with disabilities.
Describe the process for notifying parents of what protocol to follow in the event they have no digital access.
The dissemination process must be included in this documentation.
Documentation should go in the April folder since they are just now live.
