ANNUAL HEALTH QUESTIONNAIRE

The AHQ is given to every student at the beginning of the school year. It is my way getting updated information from parents, as well as, monitoring for allergies, asthma and other conditions. This form is also an important tool in helping me assess a family need for health insurance. Information is only shared with teachers if you circle YES to question number 7 giving me permission.
