The Brooklyn School requires a Health Assessment exam for all students during their 6th grade school year and must be on file with the school nurse prior to entering the 7th grade. This exam must be on a Connecticut State blue form( can print on white paper) and must include all *asterisk items:

C. *Blood Pressure
D. *HGB/HCT (blood test for anemia-iron deficiency)
E. *Postural Screening (curvature of the spine)
F. *Gross Dental
G. *Chronic Disease Assessment
H. *TB risk
I. *Vision Screening
J. *Auditory Screening
K. *IMMUNIZATIONS
Health Assessments can be offered, free of charge, to families with financial need. Please contact the school principal in this regard.
Preschoolers are required to have a new physical done every year they are in Preschool and on file to enter Kindergarten. All *asterisk items must be completed as age appropriate:

TB risk
Dental Concerns
Developmental Assessment
Chronic Disease Assessment
IMMUNIZATIONS
Children over the age of five may use the CT Blue Form.
1) If your child is planning to participate in any school sport during the upcoming school year, a physical is required in order to be eligible to participate in tryouts, practices, and games.
2) If your child has a New or Changed Health Condition, please inform the school nurse.
