Physical Requirments

GRADE 6 STUDENTS

The Brooklyn School requires a Health Assessment exam for all students during their 6th grade school year and must be on file with the school nurse prior to entering the 7th grade.This exam must be on a Connecticut State blue form and include:

A. Height

B. Weight

C. Blood Pressure

D. HGB/HCT (blood test for anemia-iron deficiency)

E. Postural Screening (curvature of the spine)

F. Gross Dental

G. Chronic Disease Assessment

H. TB risk

* Vision screenings are done in school for Grade 6

Health Assessments can be offered, free of charge, to families with financial need. Please contact the school principal in this regard.

IMPORTANT INFORMATION TO NOTE:

If your child is planning to participate in any school sport during the upcoming school year, a physical is required in order to be eligible to participate in tryouts, practices, and games.

REMINDER: If you would like your child to receive acetaminophen (Tylenol) or ibuprofen (Motrin) during the up coming school year, then a written authorization form must be signed by you and your child's physician.

ALL MEDICATIONS GIVEN AT SCHOOL MUST HAVE A PHYSICIAN'S SIGNATURE.

If your child has a special health concern, remember to speak with the school nurse.
