Report a Claim to Broadspire
Broadspire is our third party claims administrator..Â
They assist with claims that go through our Foundation Group Insurance Captive.. Â
The Captive covers the following:Â Workers Comp, General Liability & Auto
All our other policies are covered by different carriers and CSDZ would generally assist us with those claims. Â
Attached are copies of policy for Gen Liability and Workers Comp.Â
Also attached is a directory of Broadspire personnel.Â
Also attached is the file "2021-2022" which details out BHI's various policies and coverages..   Â

Once an injury or illness has been reported to the employer, the employer has 7 days to report the claim “Employers First Report of Injury or Illness” (Form 122e) to its insurance carrier and the insurance carrier has 14 days to report the “First Report of Injury” electronically to the Industrial Accidents Division. The employer must provide a copy of the form 122e to the injured worker.
