The EHS team is often asked to provide support in various way. To make things easier for everybody, the following forms have been created for the requesting employee (typically a member of middle to upper management) to provide the details of the request. Requests auto populate a Google Spreadsheet for historical data and to notify member of the safety team of the request. 

Challenge an EHS Program

This space is intended to challenge and improve any safety programs, forms, or applications that may need refined, revised, or retired. Change takes time, research, and effort from multiple people. You may be required to submit a SWOT Analysis, explained further in the Google Space to help us understand your reason for inviting change to that specific EHS topic. How to request a plan.

It is important that you provide all the requested information in order to provide you with a quality product for your PEP and for use on your project.

Once you submit the Request form with all of the required information, please contact Rory Mele at rmele@bhico.com, to ensure your request was received.

Please complete the form to the left to request a Site Specific EHS Plan. You will need to provide us with the Complete Scope of Work, exactly how you want it presented during the Project Execution Plan. You will need to review the Risk Register which lists all of the AHAs and let us know which AHAs are applicable to each item in your scope of work.


New Activity Hazard Analysis (AHA)

We have a Risk Register of all of our job tasks completed by BHI. These tasks have been analyzed and given a Risk Value and we use them to approach those tasks as safely as possible. Each task is considered for our project EHS Plans. They mirror what our JSA's look like and are intended to supplement the JSA process. 

They can serve as a safety meeting topic. They should also be reviewed by Short Service Employees and can be the subject of discussions entering a new phase of the project. 

If you don't see a job task/or hazard in our Risk Register, use this form to submit the details to create a new AHA, which will be added to our register.
