Sign up for basketball using the links below!  Be sure to complete ALL 5 STEPS!

1. General Winter Sports Online Sign up form (online form)

2. Basketball Permission Form (can be printed and delivered, or filled out online and emailed to chris@avalonschool.org)

3.  Sports Physical Form (Good for 3 years)*  (Must be printed and brought to physician)

4. Eligibility Statement (choose 4a or 4b)

4a: Paper version: MSHSL Eligibility Statement and Health Questionnaire (Good for 1 academic year)** 

4b: Online version: MSHSL Eligibility Statement and Health Questionnaire (Good for 1 academic year)** 

5. Online payment $200 suggested (can also mail or bring money in)

*Fill out student information, sign (and have your student sign it), then fax or bring to doctor.

**Go through checklist on page 1, read pages 2-9, fill out, sign, and turn in pages 10-13 

***Transfer Students: If you transferred to Avalon in the past 365 days, please fill out this form: Transfer Eligibility Form

A reminder that Basketball is a cooperative sport with High School for Recording Arts (HSRA).  Registration questions come to chris@avalonschool.org, but any other questions (scheduling, etc.) should be directed to Maureen Foss (maureen@hsra.org ) 

Basketball start date: Nov. 11th (girls) or Nov. 18th (boys/all gender)

Students signing up for this cooperative sport should be able to successfully and independently navigate to HSRA for practices.
