*Fill out student information, sign (and have your student sign it), then fax or bring to doctor. Bring fully completed and signed form to Chris or Avalon front office.
**Go through checklist on page 1, read pages 2-9, fill out, sign, and turn in pages 10-13

A reminder that Girls Volleyball is a cooperative sport with High School for Recording Arts (HSRA). Registration questions come to chris@avalonschool.org, but any other questions (scheduling, etc.) should be directed to Maureen Foss (maureen@hsra.org )
Volleyball start date: September 5th
End date: October 15th
Students signing up for this cooperative sport should be able to successfully and independently navigate to HSRA for practices.

2023 Volleyball Season Information
-Volleyball practice will start Tuesday, September 5th. Girls should arrive at HSRA gym by 4:30 to get ready and help set up.
-Paperwork must be completed before attending practice or games.
