Volleyball Registration Materials 

(Registration will close in early September)

1. General Sports Online Sign-up form (online Google form to get on the mailing list)

2. Volleyball Permission Form (make a copy of this form)

3.  Sports Physical Form (Good for 3 years)*  (Must be printed and brought to physician)

4a: Paper version: MSHSL Eligibility Statement and Health Questionnaire (Good for 1 academic year)** 

4b: Online version: MSHSL Eligibility Statement and Health Questionnaire (Good for 1 academic year)** 

5.  Online payment (can also mail or bring money in)

*Fill out student information, sign (and have your student sign it), then fax or bring to doctor.  Bring fully completed and signed form to Chris or Avalon front office.

**Go through checklist on page 1, read pages 2-9, fill out, sign, and turn in pages 10-13 

***Transfer Students: If you transferred to Avalon in the past 365 days, please fill out this form: Transfer Eligibility Form

A reminder that Girls Volleyball is a cooperative sport with High School for Recording Arts (HSRA) which is a 9th-12th school.  Registration questions come to chris@avalonschool.org, but any other questions (scheduling, etc.) should be directed to Maureen Foss (maureen@hsra.org )

Students signing up for this cooperative sport should be able to successfully and independently navigate to HSRA for practices.
