Both surveys are interviewer administered and may be completed in person or over the phone with parents. Deadline to complete this survey is November 24th, 2023.
Survey 1: Asks questions regarding housing, employment, health insurance, work/school PPE, medical history and health status, COVID-19 vaccine acceptance, food security, as well as alcohol and tobacco consumption questions during the COVID-19 pandemic.
Start of Final Parent Survey + Teacher Evaluation Survey
Starting on this date, parents can begin completing Survey 6. Survey 6 helps us learn whether the level of physical activity and nutrition at home changed in response to the implementation of the SAGE gardening curriculum.

Frequently Asked Questions (FAQ)
Who can participate in Year 3?
In Year 3 of SAGE, only new participants will be able to participate in surveys and SAGE Play Days (i.e., motor behavior, eating in the absence of hunger observations, and veggie meter). These must be new/never-before-recruited parent/child participants. That means that while previously recruited participants can still participate in SAGE Play Days, no more data will be collected from them. Existing and new teachers can continue to participate in teacher evaluation surveys.
What is an Information Booth?
An Information Booth is an event where SAGE members speak with parents of preschool children ages 3-5 about the SAGE program. Within an information booth, parents are able to enroll their children by signing a study consent form and two HIPAA forms. Parents are also able to complete two short surveys for a total of $50 in gift cards and a SAGE goodie bag.
