FIRST SEMESTER, ACADEMIC YEAR 2024-2025 (NEW STUDENTS)
For all new students:
A. To be admitted, an applicant must submit the following documentary requirements to the Registrar:
Original SF9/F138 (High School Report Card) - original copy
PSA-authenticated Certificate of Live Birth - 2 photocopies
NCAE Result (if available) - original copy
2x2 (2 pcs.) and 1x1 (2 pcs.) colored ID pictures, upper garment with collar, on white background
Certificate of Good Moral Character - 1 original copy
BIR Form 2316 / Certificate of Non-payment of Tax of Parent/s
Voter’s ID or Voter’s Certification of Parents - 1 photocopy
Barangay Certificate of Residency - original copy
Secure Eligibility to Enroll certificate from the Registrar.
Proceed to enrollment procedures in B.
B. For new and returning students:
The student will find the assigned section from the lists released by the College or Registrar.
The student shall undergo advising/evaluation of their grades based on the issued Program of Study (prospectus) and their Certificate of Grades (COG).
The student shall fill out the online registration as a pre-enrollment requirement.
The student shall proceed to the Registrar’s Office during the scheduled date of enrollment and present his/her signed clearance form.
The student shall fill out the needed date in the Registration Form (2 copies) and sign the Certificate of Registration (COR).
The Registrar’s Office shall issue an original copy of the Registration Form to the student.
