Transition Service Cards (HSS)
Human Support Services (HSS) - Research, and disseminate information regarding interagency support for key stakeholders in the WorkAbility I program at the state, regional, and local levels.
What are Transition Service Cards?
The creation of the Transition Service Cards stem from a project piloted by Chris Leroy, Region 5 Manager, and was brought to the HSS Committee, which brainstormed a list of transition areas and agencies to research and share with staff in the field.
The Transition Service Cards are meant to be a resource for all district staff that play a role in helping students with disabilities successfully transition from high school to the adult world. These cards can be downloaded and printed for distribution to parents and students at IEP meetings where transition related services and needed supports are discussed. The cards give a description of the services provided by the agency, eligibility for services, the process for applying for services, and can be personalized with your local contact information.
The Transition Service Cards have been updated effective 1/1/2016. The cards are now printable in Google Docs. You will find updated information, new internet links, and new cards. The cards do not view correctly on an iPAD. If you have any questions or concerns, please contact your HSS Chair or Region Representative.
