As part of your Graduate Assistant position, you are offered various university benefits offered to employees. Please see this website for more information about the benefits offered to graduate assistants (TAs). Not everything covered on this website is available to graduate assistants though.
Note: The time frame in August when you have to sign up for benefits is very, very short (like a few days). So you’ll need to start becoming acquainted with the information now. Immunizations Health concerns
It will take a few weeks before the university health insurance is active in September. (One usually needs to wait for the forms to be processed and the cards to come in the mail before the health care providers will allow you to make an appointment.) If you are taking any medication by prescription you may want to bring a couple of months supply with you until you are able to see a doctor and get a current prescription.
The Graduate School, nor the University, nor the Math Dept officially require a physical exam nor immunizations but University Health Services (UHS) (the Health Center on campus) requests the information so they have it on file. Visit this website to enter the information online.

*If you are an international student, you might want to check with the embassy to see if they require any immunizations or physical examination for the visa.
