Sam Stechmann also has the LaTeX version of the UW Letterhead that he's made available.

Additionally, there are at the bottom of this page a couple of files that might help you with a different version of department letterhead.

One is sampwisc.tex, a LaTeX demonstration of how to use the UW letterhead. To download it, click on the attached file. To compile it, you will also need the file wiscmath.sty, also attached.
