Descriptions of Required Forms
Employee Self-Identification Form: This form asks you basic questions about yourself and is used for Human Resources information.
Documents needed for Verification: Documents you need to bring to start your paperwork.
W-4 Employee's Withholding Allowance Certificate: This form is a tax form that allows you to determine how much tax will be taken from your paycheck.
Authorization for Direct Deposit of Payroll: This form is where you indicate where your paycheck is deposited. You are required to participate in direct deposit. Each paycheck will be deposited electronically into the bank account that you specify on this form.
Group Health Insurance Application and Change Form: As a teaching assistant in the Math Department you are offered a health care plan as part of your benefits package. This form allows you to apply and enroll in one of the health care plans offered by the Math Department. Information about each plan can be found here.
*International students must complete the following documents in addition to the ones listed above:
International Student SHIP Waiver Form: International students residing in the United States are required by the Federal Government to enroll in some sort of health plan. If you fail to sign up for a Math department offered plan you will be automatically enrolled in the University's SHIP plan and be charged at a higher rate than what the Math department offers. International students are required to fill out this form to indicate that they have enrolled in an Math department offered or private health care plan. You must do this or you will be charged a lot of money!!!
International Student with Dependent Affidavit for Insurance Purposes: If you are an international student with a spouse or any other dependent who does not have a Social Security Number you will need to fill out this form.

Most graduate students in the Math Department at the University of Wisconsin – Madison are supported as Graduate Assistants through Teaching Assistantships or Research Assistantships and thus are considered employees of the UW.
As new employees of the UW, you will need to fill out some Human Resources (HR), Payroll, and Benefit forms before you can work and receive your paycheck. This section contains information about getting set up as an employee in the Math department.
